Our healthcare program was founded by Nurse Practitioners who believe that strengthening the lives of those who have been systematically excluded is essential to restoring communities. We are a critical access point for many to start on their journey to healthy independence.

How do I get care?

Please bring eligibility forms with you for same-day verification

Confirm Eligibility

  • Experiencing homelessness or Housing Instability
  • Uninsured with Financial Hardship
  • Georgia Medicaid
  • Types of Acceptable Documentation: Homeless Verification Letter, Wage Verification Letter, Last Year’s W2, 2 Recent Paystubs

Know Before You Go

  • Walk-in for general clinical services
  • Appointments are available for mental health
  • Clients are seen in the order they arrive, severity of visit, and based on provider availability
  • Clinic may stop accepting clients if waiting room is full
  • We are closed on weekends and major holidays

No onsite parking. Street parking is available on John Wesley Dobbs Rd. Rideshare or public
transit is encouraged.
