ATLANTA, Oct. 8, 2025,— Our House, Inc., a leading Atlanta-area nonprofit providing essential support to families experiencing or at risk of homelessness, has been recognized by the American Heart Association and American Medical Association for its commitment to improving blood pressure (BP) control rates, earning Silver+ recognition as part of Target: BP™.
The Silver Plus achievement award recognizes organizations that have demonstrated a commitment to improving blood pressure control through evidence-based practices for BP measurement and treatment.
High blood pressure, or hypertension, is a leading risk factor for heart attacks, strokes and preventable death in the U.S. There are 122.4 million U.S. adults living with hypertension. That is nearly half of all adults in the country.[1] Unfortunately, just a quarter of them have their BP under control, making both diagnosis and effective management critical.[2] In the U.S., heart disease and stroke are the No. 1 and No. 5 causes of death, respectively, and stroke is a leading cause of disability.
In Atlanta, people facing housing instability or lacking access to insurance are at even higher risk, often going years without a blood pressure check or a healthcare visit, which makes interventions like those offered by Our House especially critical.
Last year, Our House received the Silver Target: BP achievement award for its dedication to improving blood pressure control among adult patients! Silver+ is a newer designation in the Target: BP recognition structure reserved for organizations that met Silver-level evidence-based activity attestation and demonstrated additional improvement or expanded adherence to the program’s evidence-based practices.
“This year’s award reflects our deep commitment to both quality improvement and to our community,” said Tyreesha Hubbard, DNP, CPNP-PC, Chief Clinical Officer, Our House Health. “For many of the people we serve, Our House is often their only access point for preventive care. By offering express blood pressure visits, same-day exams, lab testing, and medications that help people avoid costly ER visits; and by expanding self-monitoring programs that put knowledge and tools directly into clients’ hands, we’re transforming potential medical crises into manageable, long-term, lifesaving care.”
Our House provides free comprehensive healthcare, among other services, as part of its broader mission to support Atlanta families experiencing homelessness and those with low incomes and without insurance. In 2024, Our House Health served nearly 2,000 patients through more than 4,700 visits. These services are designed to meet people where they are, removing cost, insurance, and transportation barriers that often keep vulnerable families from seeking treatment.
Target: BP is a national initiative formed by the American Heart Association and American Medical Association in response to the high prevalence of uncontrolled blood pressure. The initiative aims to help health care organizations and care teams, at no cost, improve blood pressure control rates through an evidence-based quality improvement program and recognizes organizations, like Our House, that are committed to improving blood pressure control.
“By committing to helping more people in Atlanta control their blood pressure and reduce their risks for future heart disease and stroke, Our House is taking a key step to helping more people live longer, healthier lives,” said Wanpen Vongpatanasin, M.D., volunteer member of the Target: BP Advisory Group and liaison to the American Heart Association Hypertension Council. “Our House’s participation in the Target: BP initiative shows its dedication to turning clinical guidelines into lifelines for patients and their families.”
