Our House Health Earns Three American Heart Association Recognitions for Cardiovascular Care
2026 honors highlight the Atlanta nonprofit’s commitment to high-quality healthcare and evidence-based blood pressure, type 2 diabetes and cholesterol management
ATLANTA — Our House, Inc. announced today that its comprehensive healthcare program, Our House Health, has earned three 2026 American Heart Association recognitions for its commitment to high-quality blood pressure, type 2 diabetes and cholesterol management.
Our House Health received the 2026 Target: BP Silver+ Achievement Award, recognizing its commitment to evidence-based practices supporting accurate blood pressure measurement and hypertension management. The organization also earned the 2026 Target: Type 2 Diabetes Participant Award for its commitment to improving quality of care for patients with type 2 diabetes and associated cardiovascular risk factors, and the 2026 Check. Change. Control. Cholesterol Participant Award for its commitment to improving cholesterol awareness, detection and management.
High blood pressure, type 2 diabetes and high cholesterol are major risk factors for heart disease and stroke. According to the American Heart Association’s 2026 Heart Disease and Stroke Statistics Update, an estimated 130.6 million U.S. adults, nearly half the adult population, had some form of cardiovascular disease between 2021 and 2023. Early detection and ongoing treatment can reduce the risk of serious complications, but consistent access to care remains a challenge.
In Georgia, more than 1.3 million residents lacked health insurance in 2024, according to U.S. Census Bureau data included in Georgia Health Initiative’s 2026 report, Insights on Medicaid in Georgia. For households already facing financial hardship or housing instability, the cost of routine care and medications can make it difficult to manage health conditions before complications arise.
“Atlanta has made important investments in helping families build stability. We need to be just as deliberate about protecting the progress they make,” said Tyese Lawyer, President and CEO of Our House. “An unexpected diagnosis can create financial strain for most households. For families without insurance or the financial means to absorb treatment costs and lost income, a health setback can jeopardize housing and undo years of progress. If we want to reduce poverty and expand opportunity across generations, access to high-quality preventive healthcare must be part of Atlanta’s long-term investment strategy.”
Our House Health works to close critical gaps in healthcare access by providing comprehensive free healthcare to people with low incomes who are uninsured, people experiencing or at risk of homelessness, and people who have Georgia Medicaid coverage. Services for children and adults include primary and preventive care, reproductive healthcare, mental and behavioral healthcare, chronic disease management, laboratory services, school and employment exams, and care coordination.
The program also provides medication and transportation assistance, certified interpretation services and referrals for specialized care to help patients overcome barriers to treatment. These supports are particularly important for managing chronic conditions that require consistent monitoring, medication management and follow-up care.
“Clinical standards shouldn’t change because a patient lacks insurance or stable housing. What has to change is how we deliver care to patients facing those barriers,” said Tyreesha Hubbard, DNP, CPNP-PC, Chief Program Officer at Our House. “Effective management of high blood pressure, type 2 diabetes and high cholesterol depends on early identification, appropriate treatment and consistent follow-up. Our clinical team understands how easily that continuity can be disrupted and works to ensure patients receive the care they need without compromising quality. I’m exceptionally proud of the expertise, compassion and persistence they bring to this work, and pleased to see their dedication recognized nationally.”
The 2026 recognitions build on Our House Health’s 2025 Target: BP Silver+ Achievement Award, reflecting a sustained commitment to evidence-based care and a broader focus on cardiovascular health.
